Sleepy Diary and Questionnaire
To enable your sleep consultant to prepare and plan your individual sleep programme, it is vital that these documents are received by your sleep consultant before 3 pm on the day prior to your initial consultation. If you are having problems forwarding these documents to us, please contact the main Millpond office as soon as possible. Failure to contact us or provide these documents will delay your consultation and may incur the cancellation fee.
Duration of Packages
Although we endeavour to accommodate flexibility into the duration of our packages, in exceptional circumstances the core and siblings’ sleep packages will extend no longer than two months.
If you have an unused review appointment remaining in your package within the time frame outlined above, you will need to contact your sleep consultant to arrange your final appointment. If we do not hear from you, we will assume all is well and discharge you. Unused reviews cannot be taken up beyond two months (core and siblings’ packages) from your initial consultation appointment.
Cancellation or Rescheduling
We ask for a minimum of 48 hours (two working days) notice of cancellation from the date of your initial consultation as we are unable to refill your appointment at short notice. Given this notice a full refund less £10 administration fee will be made.
In the event that less than 48 hours notice is given, a £30 cancellation charge will be made on all cancellations.
Parents who have used Millpond previously, perhaps for an older child, qualify for a 10% discount off our core, and siblings’ sleep packages.
